The report below lists all North Section participants' Athlete Application for Participation (medical) and Class A Volunteer Form  with their expiration dates (see sample screen capture of report below).

You can follow the instructions below to find the information you may be looking for in the report. <open or download report report now>

Note - username and password must be entered in lowercase letters!

To search by last name: Use Edit/Find or the Shift +  Control + F keys to open the find dialog box then type the last name of the person you are looking to find.

Search for Local Program page: You can also scroll through the report pages to find the local program name and then look through the list for the person. Athletes are listed first then volunteers (chaperones, coaches, unified partners).

Notes:
1. Some participants have multiple forms on file including ones that are expired. This list will show the most current expiration date for those people so you will know that they are OK to participate.

2. Participants are grouped by Primary Local Program not by the local program for a specific sport. So if you have scrolled to your local program and do not see a participant, he or she may participate with another local program that is designated as their primary local program. Please use the 'Search by Last Name' method described at top to find the participant.
